	If you are not a Singapore Citizen or Singapore PR, please do not use this service unless your National Immigration Authority accept a Singapore executed Deed Poll.

	For Singapore PR, you will only be able to change your name in your Singapore Blue NRIC but not your name in your Foreign Passport. You are strongly advised to check with your National Immigration Authority on issues relating to your Foreign Passport and validity of a Singapore executed Deed Poll.

Important Note & Declaration

	Important Notes for Change of Minorⁱ's Name via Deed Poll
	From 1 June 2007, Immigration Checkpoints Authority (ICA) requires one parent to complete a form to declare that the Minor’s other parent has also consented to the name change, in addition to producing the Deed Poll. If the other parent is uncontactable, the parent who signed the Deed Poll must state this information on the Declaration Form accordingly

	By applying for a Deed Poll to change your Minor Child's Name, you DECLARE that you have obtained the other parent's consent, OR the other parent is uncontactable, and you are aware that you are required to make the above declaration to ICA when you attend at ICA.

	Once you have signed you Deed Poll, you will keep your original Deed Poll, and we will provide a photocopy of your Deed Poll.

	If you require a copy of your Deed Poll to be certified true by our Lawyer, our fee is S$10.00 (net) for each certified true copy in addition to our Standard Charges. If you don't need certified true copies of your Deed Poll, you need not apply.
